Overview

Assist clients with intake assessments and navigate healthcare systems.

You have:

  • Associate's or Bachelor's degree in social work, public health, or related field of study preferred.
  • Relevant professional experience with similar populations and/or client needs strongly preferred.
  • Strong relationship building, diplomacy, and networking skills required.
  • Demonstrated success working and communicating effectively in a multi-cultural environment required.
  • Fluency in English required, both spoken and written; proficiency in languages spoken by the client group preferred, especially in Spanish.
  • Proficient in Microsoft Office applications (Word, Excel, Outlook) required.
  • Valid driver's license and access to a personal, insured vehicle preferred but not required.

Major Responsibilities:

Responsibilities include, but are not limited to:

•Assist in intake assessments, enrolling clients, and determining eligibility at intake; if clients are found to be ineligible for services, provide relevant referrals.

•Assist clients in navigating healthcare and social service systems, which may include helping clients apply for benefits and attend and schedule appointments.

•Assist in mapping out community resources and making client referrals to community partners.

•Provide clients with extended orientation on a variety of topics such as women’s health, navigating transportation systems, and navigating physical healthcare systems at the local level.

•Conduct community outreach to service providers to ensure client needs are met.

•Support ICM and SCM teams in response to client crises.

•Assist clients and ICM and SCM team members in requesting financial assistance when applicable and track the financial assistance received in the client’s case file.

•Maintain up-to-date documentation in client’s case file to ensure services are properly documented throughout the client’s service period; document all services provided in IRC’s database, Efforts to Outcomes (ETO).

•Attend relevant trainings and meetings at the local and national level.

•Use personal, insured vehicle and/or public transportation (where available) to travel and transport clients and materials as needed throughout the service delivery area.

•May train and lead the activities of volunteers and interns.

•Other duties as assigned.

Job Requirements:

Education: Associates or Bachelor’s degree in social work, public health, or related field of study preferred.

Work Experience:

•Relevant professional experience with similar populations and/or client needs strongly preferred.

•Self-starter with excellent problem-solving skills combined with the proven ability to prioritize and manage time effectively.

•Attention to detail and accuracy in work product.

•Fluent in English required, both spoken and written; proficiency in language(s) spoken by client group preferred. Strong preference for proficiency in Spanish.

•Proficient in Microsoft Office applications (Word, Excel, Outlook); ability to use the internet and other digital tools for data entry, research, and problem-solving purposes.

•Preferred but not required: Valid driver’s license and access to a personal, insured vehicle.

Working Environment:

•A combination of standard office environment, remote work, and ‘field’ time within the service delivery area to perform the above outlined responsibilities.

•May require occasional weekend and/or evening work.

Compensation: Posted pay ranges apply to US-based candidates. Ranges are based on various factors including the labor market, job type, internal equity, and budget. Exact offers are calibrated by work location, individual candidate experience and skills relative to the defined job requirements.
