Overview

Provide health care services and support to refugees and immigrants in Sacramento.

You have:

  • Associate degree with an emphasis in social work, public health, or related field of study required; bachelor's degree preferred.
  • Minimum of 2 years of related experience required, with a preference for experience working with refugee populations, low-income individuals, homeless individuals, hard-to-reach communities, and minority groups.
  • Experience in case management, community outreach, and connecting individuals to essential health and social resources.
  • Strong written and verbal communication skills, including the ability to make presentations.
  • Bilingual proficiency is required to effectively serve a diverse client population and overcome language barriers.
  • Proficiency in Microsoft Word, Excel, and PowerPoint; experience with data tracking systems is highly preferred.
  • Valid driver's license and access to a personal, insured vehicle.

Major Responsibilities:

Responsibilities include, but are not limited to:

- Work with IRC Departments and programs in Sacramento to ensure clients receive health care services, enrollment, follow-up care, and referrals. - Provide case management and targeted support services to refugees and immigrants, including individual coaching, health navigation, benefits assistance, access to community services, and ongoing care coordination. - Provide psychosocial support to individuals, families, and the community through wellness activities, including screening tools, client support groups, brief interventions, and Mental Health First Aid Trainings. - Create and maintain safe spaces for open discussions on mental health, wellbeing, and victimization, including gender-based violence, while integrating trauma-informed care for immigrants, refugees, and individuals affected by displacement. - Offer bilingual and culturally congruent services to ensure comprehensive language access for clients. - Advocate for individuals by addressing the unique challenges of mental health, victimization, and trauma, with a focus on inclusivity and the intersection of immigration experiences. - Create and maintain safe spaces for open discussions on mental health, wellbeing, and victimization, including gender-based violence, while integrating trauma-informed care for immigrants, refugees, and displaced individuals. - Schedule clients for Refugee Health Clinic (RHC) appointments, follow-up to ensure enrollment in health benefits/services, and connect clients to other resources. - Conduct needs assessments and develop care plans to address clients' health care needs. - Facilitate community education activities, including presentations on cultural orientation, safety and diseases prevention workshops, wellness workshops, and local community outreach. - Ensure accurate case notes, tracking screenings, services, referrals, and outcomes, and assist with reporting for relevant grants. - Assist clients in navigating health care services, including scheduling appointments for OB-GYN, dental care, and mental health services. - Provide clients with extended orientation on health care systems, insurance enrollment, transportation, and interpretation services. - Maintain up-to-date documentation in client files and IRC’s database, Efforts to Outcomes (ETO). - Support in translating written materials, resources, and documents to ensure accessibility for non-English speaking clients. - Develop and maintain partnerships with organizations to ensure clients have access to services that meet their needs, such as safe housing, food, and legal services. - Oversee volunteers and interns involved in assigned projects. - Participate in all program meetings, staff development activities, and engage as a member of the team. - Offer clients timely information on and referral to community service providers and resources, as needed. - Other related duties as assigned.

Internal: The Health Caseworker will work as a member of the Community and Wellness team. This position will also develop working relationships with IRC Sacramento other departments staff and will collaborate with internal programming to support achievement of program goals.

External: The Health Caseworker will develop working relationships and support referrals systems with community service providers under the topic areas of health, mental health, benefits assistance, and community services. This includes, but is not limited to, insurance providers, the Department of Human Assistance, local healthcare clinics, and mental health providers.

Job Requirements:

Education: Associate degree with an emphasis in social work, public health, or related field of study required; bachelor’s degree preferred.

Work Experience:

- Minimum of 2 years of related experience required, with a preference for experience working with refugee populations, low-income individuals, homeless individuals, hard-to-reach communities, and minority groups. - Previous experience in multicultural settings and demonstrated ability to communicate effectively with individuals from diverse backgrounds. - A background in health or mental health services is required, focusing on providing non-clinical support and services. - Experience in case management, community outreach, and connecting individuals to essential health and social resources. - Ability to assess client needs, offer guidance, and provide referrals to appropriate services, including counseling and social support. - In-depth knowledge of community resources and support networks, particularly for underserved populations such as immigrants, refugees, and minority groups. - Proven experience advocating for clients’ needs within health and social service systems to ensure access to care and services. - Strong written and verbal communication skills, including the ability to make presentations. - Proficiency in Microsoft Word, Excel, and PowerPoint; experience with data tracking systems is highly preferred. - Bilingual proficiency is required to effectively serve a diverse client population and overcome language barriers. - Exceptional attention to detail with strong organizational skills to manage and track large volumes of information. - Commitment to teamwork and collaboration across various IRC programs. - Highly motivated, with the ability to work independently, develop new programs, and handle multiple tasks efficiently.

Working Environment:

- A combination of standard office environment, remote work, and ‘field’ time within the service delivery area to perform the above outlined responsibilities. - May require occasional weekend and/or evening work.

Compensation: Posted pay ranges apply to US-based candidates. Ranges are based on various factors including the labor market, job type, internal equity, and budget. Exact offers are calibrated by work location, individual candidate experience and skills relative to the defined job requirements.
