Job Summary

The APA Health Services Caseworker will be responsible for coordinating medical and behavioral health services to APA clients. The Health Services Caseworker will be working closely with the IRC caseworkers and the Tucson community health providers to coordinate medical screenings and health services for new Afghan arrivals under APA program. Under the direction of the Health Program Manager, the Health Services Caseworker is responsible for developing partnerships with community organizations and ensuring access to quality, culturally and linguistically appropriate physical and mental health services for IRC clients.

Major Responsibilities

· Coordinate delivery of initial health screening for all newly arrived clients under the APA program;

· Coordinate delivery of medical case management services for clients enrolled in APA and other programs with medical and other service providers within Tucson Community;

· Outreach to local medical providers to enhance the health care options and provision of eligible clients;

· Coordinate with the Health team to arrange medical appointments for new arrivals needing immediate medical assistance;

· Coordinate with the health team and other program Supervisors to provide client-centered services;

· Provide transportation for clients to appointments;

· Provide specialized assistance and advocacy for clients needing assistance in accessing health care coverage which may not be within Medicaid;

· Ensure appropriate documentation, records, and tracking tools for program case files and reporting. Create tools as necessary for staff and conduct regular case file audits;

· Participate in monitoring and evaluation of programs within the APA Casework team;

· Monitor ongoing progress of outcomes for clients with caseworkers assigned, randomly meeting with clients monthly to review their cases;

· Provide recommendations for program and service improvements;

· Participate in all program meetings, staff development activities, and fully engages as a member of the team.

· Follow all IRC policies, procedures and protocols including adherence to the IRC Way and Child Safeguarding.

Key Working Relationships

Position Reports to: Health Program Manager

Position Directly Supervises: interns and volunteers within the program

Other Internal and/or External Contacts

Internal: IRC Tucson Staff, HQ Resettlement Team

External: Community and Stakeholder partners, Arizona State Refugee Health Coordinator, and other medical providers.

Job Requirements

· Undergraduate degree in public health, social work or related field or equivalent work experience.

· Two+ years of professional work experience; preferably in a non-profit or social services environment; with a minimum of one-year direct casework experience.

· Lived experience as refugee or immigrant. In lieu of lived experience, experience working with refugees and/or immigrants; cross-cultural or overseas experience accepted.

· Proficiency in written and spoken English, bilingual ability preferred in Dari, Pashto and/or Urdu. Cross-cultural understanding and strong communication skills, both written and verbal.

· Computer proficiency with an emphasis on Microsoft Office applications (Word, Excel, Teams, Outlook).

· Highly organized self-starter with shown success prioritizing multiple tasks, making critical decisions, and delivering results in a fast-paced environment with tight deadlines.

· Ability to document client interactions, services provided, maintain updated and accurate case files, and prepare accurate and timely reports.

· Strong attention to detail with the ability to prioritize and manage time effectively.

· Flexibility with demonstrated ability to maintain responsibility for multiple tasks in the context of a diverse, fast-paced work environment.

· Strategic, creative, problem solver capable of working well independently and as a part of a team.

· Must have a valid driver’s license, active insurance policy, access to reliable transportation and the ability to travel regularly throughout the service delivery area (home visits, agency visits, service provider visits etc).

· In accordance with Arizona State Law, must possess or be eligible to receive a fingerprint clearance card.

Working Environment:

· A combination of standard office environment, remote work, and ‘field’ time within the service delivery area to perform the above outlined responsibilities.

· May require weekend and/or evening work.
