Background and Justification

Madagascar faces major health challenges. Respiratory infections, diarrheal disease, malaria, malnutrition, maternal and neonatal mortality, poor hygiene and sanitation, and limited water infrastructure are a burden on Madagascar’s families, communities, health system, and economy. With limited access to basic health services, every day 100 Malagasy children die primarily from common and preventable illnesses and 10 Malagasy women die from complications related to pregnancy and childbirth. The country’s high maternal mortality rate has essentially remained unchanged for more than two decades.

Across Madagascar utilization of health services is low; over the past several years, use of health services has remained at about one-third, the key reasons being the cost of and distance to services.

Despite significant progress toward achieving its Millennium Development Goals, Madagascar was unable to meet many of its targets. The Government of Madagascar has identified Primary Health Care (PHC) as a priority for improving health outcomes and is therefore updating its National Health Policy and Strategy. Additionally, there is no existing data bank for health financing, and the human resource capacity within the country to implement health costing exercises, modeling with LiST, and Bottleneck analysis with EQUIST is rapidly waning out.

UNICEF Madagascar seeks to gather evidence on the current bottlenecks within the PHC system, estimate the future costs and required financing, as well as the expected health benefits of equitable access to community health interventions. UNICEF also seeks to develop a health financing data bank and a pool of personnel within the Ministry of Health capable of implementing health costing exercises.

Purpose

Technical assistance for costing of comprehensive Primary Health Care (PHC) model and Lives Saved Tool (LiST) analysis in Madagascar.

How can you make a difference?

Scope of Work

  1. Costing of a conceptual model for a scalable and sustainable comprehensive community-based PHC model with a clearly defined governance structure and strong accountability framework
  2. Introduce PHC costing in the areas of i) improving availability, timeline, and quality of health information at the subnational level; ii) improving MSANP capacity to analyze data (including the coverage and equity assessment, bottleneck analysis) and use it to inform equity-focused planning, decision making, monitoring, and budgeting; iii) ensuring citizens’ voice is incorporated into planning, monitoring, and budgeting process and that functional feedback loops exist between the health system and communities; iv) supporting continuous quality improvement at the facility level as well as community service delivery by CHWs
  3. Conducting Lives Saved Tool (LiST) analysis in the areas of maternal, newborn, and child health.
  4. Lead the creation of a pool of experts at the national level with the capacity for costing using the OHT and modeling with LiST within the ministry of health
  5. Carry out a scoping analysis on the feasibility of including a health financing module within the training curricula of Public Health training institutions ins Madagascar
  6. Setup a national health financing data bank

Deliverables outlined below, are to be due on a timeline to be agreed upon at the start of the contract. A time-bound work plan for the deliverable in Table 1 below will be developed by the consultant in the 1st week of the assignment.

Payment Schedule

Payment will be made upon the satisfactory quality completion of deliverables as scheduled and described in the Payment Schedule.

Travel (local) costs will be reimbursed as actual and as approved. Due to COVID-19 and the terms of this assignment, no international travel is anticipated for this assignment.

Administration and Reporting

The consultant will work under the supervision of the Health Specialist, Health Systems Strengthening who will be responsible for the general coordination and scheduling and in close collaboration with the Chief of Health. The weekly meeting will be held between the consultant and his supervisor to discuss progress and adjustments. The Madagascar Health team will provide inputs along with the different processes and will be consulted. The consultant will be provided with a desk office but will be using his/her own personal computer. The regular office hours will apply to the consultant. The payments will be made subject to the submission of progress reports.

To qualify as an advocate for every child you will have…

  • An advanced university degree (Master’s or higher) in Public Health, Social Science, Health Economics, or in a relevant field. *A first University Degree in a relevant field combined with 2 additional years of professional experience may be accepted in lieu of an Advanced University Degree.
  • A minimum of 8 years of relevant professional experience in health sector program analysis, management, and leadership.
  • Prior experience with National strategic plan costing, RMNCAH Investment Case, and CHPCT use in the Madagascar context is highly desirable.
  • Proven experience working with/liaising with MoHP and other key stakeholders engaged in community health.
  • Good understanding of the iCCM program, including national-level strategic planning and implementation across the health system tiers.
  • Strong analytical, negotiation, oral, and written communication skills
  • Effective presenter including the ability to adapt the message and visual aids for multiple audiences to deliver concise, impactful presentations of primary health care interventions.
  • Effective facilitator with proven ability to engage and train a group of individuals at the national level and for front line health workers as well
  • Developing country work experience and/or familiarity with emergencies is considered an asset.
  • Fluency in French is required. Written and spoken fluency in both French and English are desirable.
