Patient Safety Consultant

Result of Service By 31 December 2023 the consultant will: - Submit all cause mapping investigation documentation for all RCA (cases) using the template given by CGS within two weeks of completing the RCA follow up. - Submit a final report summary of system or process trends identified across all RCAs which will include a summary of the feedback and recommendations collected from the causal mapping. - Submit a final report presentation of the final report summary (see above) in powerpoint.

Duties and Responsibilities This position is in the Clinical Governance Section of the Division of Healthcare Management and Occupational Safety and Health, Office of Support Operations, Department of Operational Support. The Clinical Governance team is responsible for development and implementation of Healthcare Quality and Patient Safety Standards. The incumbent will be reporting to the Senior Medical Officer, Clinical Governance Section (CGS).

The Division of Healthcare Management and Occupational Safety and Health provides expert healthcare management and OSH services to all UN secretariat entities (Field missions, Offices Away from Headquarters and Economic Commissions), the New York based agencies funds and programs, as well as providing strategic leadership of whole-of-system medical issues through the UN Medical Directors Network.

The Patient Safety Consultant (PSC) is responsible for supporting the organization’s overall goal of becoming a high-reliability organization by facilitating patient safety and quality improvement initiatives designed to reduce error and improve patient outcomes. Under the direct supervision of the Senior Medical Officer, the consultant will: - Facilitate root cause analysis (RCA) investigation meetings with appropriate teams for sentinel events and work to assure that corrective actions are proposed in a timely manner. - Document cause and effect, solutions and timeline for all RCAs - Conduct Root Cause/Common Cause analysis reports for sentinel events that include but are not limited to: 5-Why, cause map, solutions, matrix and timelines as appropriate. - Submit complete cause mapping investigation documentation for sentinel events using the framework provided by CGS. - Foster a non-punitive culture of continuous learning and improvement by reviewing specified cases, identifying trends in near-miss and actual events, and using Lean methodologies to identify trends in contributing factors of events. - Assist in the creation and implementation of a clinical adverse event reporting system.

Qualifications/special skills Bachelor’s Degree Required, RN preferred; Master’s Degree Preferred; CPHQ Preferred At least 7 years of prior work experience in direct patient care At least 7 years of quality and patient safety in healthcare with emphasis on sentinel event/causal analysis. Demonstrated experience and skill in facilitating root cause and common cause analysis, clinical event timeline. Experience with UN peacekeeping field hospitals. Experience working in international organizations is an asset. Ability to work independently with minimal supervision is required. Prior consulting experience is an asset.

Languages English required; second UN working language is an asset.
