Job Overview/Summary: The IRC in Charlottesville’s Preferred Communities Intensive Case Management (ICM) program assists with the long term, successful integration of newly arrived refugees and other humanitarian entrants with medical and mental health health and other needs. The ICM Coordinator provides and coordinates holistic case management services, frequently focused on healthcare navigation, ensuring that clients are connected to necessary services and resources both within and outside of IRC’s programs. They might supervise ICM Casework Assistants, Bachelor of Social Work (BSW) and/or Master of Social Work (MSW) level interns and the Healthy Futures AmeriCorps Member and collaborate closely with colleagues internally to best serve refugee families.

Major Responsibilities: Responsibilities include but are not limited to:

Case Management and Referrals:

- Provide trauma-informed comprehensive case management for vulnerable refugees, asylees, and SIVs to help them feel safe, empowered, and stable. - Complete individualized client intakes, including orientation to services, assessment of client needs, and development of client-specific safety plans. - Develop individualized service plans for each client with an emphasis on the client’s strengths, barriers and needs. Assist clients to set and achieve goals leading to self-sufficiency, well-being, and community integration. - Connect clients to community resources including physical and mental health services, public benefits, transportation, low-income housing, social and community support, legal services, food access, childcare resources, etc. - Identify appropriate medical providers, community partners, and other social service agencies and advocate for refugees to have full, ongoing access to medical and mental health services. - Mentor, train, and provide technical orientation on ICM services to new and existing staff. - Handling logistics/admin for the entire team such as team’s weekly schedule, making sure client’s assessments are completed in a timely manner, maintaining and updated tracking system for clients’ referrals, admission, close out and number of clients served. - Facilitate regular meetings with the team to assign cases and set logistical and related goals for the week.

Partnerships and Community Engagement:

- Build and maintain partnerships with external community service providers to improve client access. Assist clients in accessing ongoing care. - Lead partnerships and serve as key liaison with youth and schools. - Establish clear referral systems with community partners, both for sending and receiving referrals. - Participate in office-wide and ICM-specific meetings, trainings, and staff development activities. - Support coworkers in responding to the urgent or unanticipated needs of vulnerable clients. - Represent IRC at community meetings and events, build relationships with local stakeholders and expand IRC’s visibility among community partners. - Provide oversight to ICM Casework Assistants, undergraduate, graduate, and other student interns and volunteers as well as AmeriCorps members.

Documentation and Reporting:

- Maintain excellent case file quality by ensuring complete and timely documentation of intakes, assessments, service plans and case notes. - Monitor and track service delivery to ensure compliance with program guidelines. Complete and submit client assessments at required intervals to track client’s progress toward service plan goals. - Prepare, distribute and document ICM clients’ direct assistance and donations. - Complete reports as required by funders and/or donors. - Comply with and remain up-to date on all policies, procedures, and best practices of the agency. - Other related duties as assigned.

Internal and/or external contacts:

- Internal: coordinate closely with youth program specialist, caseworkers, employment specialists - External: Healthcare providers, social service agencies and area schools

Job Requirements:

- Bachelor’s Degree required; preferably in public health, social work, human services, or related field; master’s degree in a human services field------- preferred. - At least two years’ case management experience required. - Experience working with refugees, victims of human trafficking, immigrant populations or vulnerable populations strongly preferred. - Understanding of Virginia social services, health insurance and medical systems preferred. - Proven ability to balance direct client services with documentation and other compliance measures. - Aptitude for working through intense mental health and/or medical situations with clients, often in an emergency. - Demonstrated success working and communicating effectively in a multi-cultural environment. - Proven ability to contribute both independently and as a key team member. - Self- starter with excellent problem-solving skills combined with the proven ability to multi-task, prioritize duties, and manage time effectively. - Fluent in English, both spoken and written; bilingual ability in one of the predominant languages of the local client base is desired: Spanish, Arabic, Swahili, French, Dari, or Pashto. - Proficient in Microsoft Office applications (Word, Excel, Outlook) - Valid driver’s license, access to a reliable vehicle with current insurance.

Working Environment:

- A combination of standard office environment and ‘field’ time in and around the service delivery area to perform the above outlined responsibilities. - May require occasional weekend and/or evening work.

Compensation: Posted pay ranges apply to US-based candidates. Ranges are based on various factors including the labor market, job type, internal equity, and budget. Exact offers are calibrated by work location, individual candidate experience and skills relative to the defined job requirements.
