The Health Services Caseworker will be responsible for coordinating medical and behavioral health services for Ukrainian new arrivals. The Health Services Caseworker will be working closely with IRC caseworkers across programs and local health providers to coordinate health services. Under the direction of the Health Promotion Supervisor, the Health Services Caseworker is responsible for ensuring access to quality, culturally and linguistically appropriate physical and mental health services and health education for IRC clients. The Health Services Caseworker will provide health literacy training to clients to increase self-sufficiency and ability to advocate for equitable medical services. This position reports to the Health Promotion Supervisor.

Major Responsibilities

- Coordinate delivery of initial health screening and health services for enrolled clients. - Coordinate delivery of medical case management services for clients based on plans of care prescribed by health professionals. - Provide health literacy, health navigation, and health education sessions with clients. - Outreach to local medical providers to enhance the health care options and provision of refugee and asylee clients. - Provide transportation or coordinate transportation for clients to medical appointments. - Provide specialized assistance and advocacy for clients needing assistance in accessing health care coverage. - Ensure appropriate documentation, records, and tracking tools for program case files and reporting. - Monitor ongoing progress of outcomes for clients with caseworkers assigned, meeting with clients monthly to review their cases. - Provide recommendations for program and service improvements. - Comply with all IRC policies, procedures and protocols including adherence to the IRC Way and Child Safeguarding.

Internal: Senior Health Program Manager, HQ Resettlement Team

External: Community and Stakeholder partners, religious and faith-based communities, DES, law enforcement, schools, Banner University Medical Center, Pima County Health Department, Arizona State Refugee Health Coordinator, and other medical providers.

Job Requirements:

**Education****: Undergraduate degree in a related field or 2 years of experience in health-related case management or health promotion position working with a minority population, is preferred.

Work Experience: Two+ years of professional work experience; preferably in public health, non-profit, or social services environment

Demonstrated Skills and Proficiencies:

- Experience working with refugees and/or immigrants; cross-cultural or overseas experience accepted. - Knowledge of the US Healthcare system - Experience coordinating health services or case management experience. - Computer proficiency with an emphasis on Microsoft Office applications (Word, Excel, Teams, Outlook). Proven experience in web-based data entry systems, with both data extraction and design preferred. - Highly organized self-starter with proven success prioritizing multiple tasks, making critical decisions, and delivering results in a fast-paced environment with tight deadlines. - Strong attention to detail with the ability to prioritize and manage time effectively. - Flexibility with demonstrated ability to maintain responsibility for multiple tasks in the context of a diverse, fast-paced work environment. - Strategic, creative, problem solver capable of working well independently and as a part of a team. - Demonstrated understanding of coaching and motivational interviewing - Valid driver’s license and access to a personal, insured vehicle. - In accordance with Arizona State Law, must possess or be eligible to receive a fingerprint clearance card.

Language Skills: Fluency in written and spoken English, bilingual ability preferred in Ukrainian or other refugee/immigrant language used in Arizona. Cross-cultural understanding and strong communication skills, both written and verbal.

Working Environment:

- A combination of standard office environment, remote work, and ‘field’ time within the service delivery area to perform the above-outlined responsibilities. - May require occasional weekend and/or evening work.

Posted pay ranges apply to US-based candidates. Ranges are based on various factors including the labor market, job type, internal equity, and budget. Exact offers are calibrated by work location, individual candidate experience and skills relative to the defined job requirements.
