Indonesia has charted remarkable progress on its path toward universal health coverage (UHC). Health insurance coverage has rapidly expanded to almost 90 percent of the population and the share of out-of-pocket (OOP) expenditures has decreased by nearly 24 percentage points since 2013, after the introduction of Jaminan Kesehatan Nasional (JKN) or National Health Insurance in 2014. Despite these major achievements, several challenges remain, especially in lowering maternal mortality rates, reducing stunting prevalence, and curtailing tuberculosis. Regional and income-related inequalities in health outcomes also persist, highlighting the importance of good governance and health information systems to better target resources.

The Indonesia health and nutrition team in the Jakarta office is engaged in supporting the health sector and the social security apparatus through several related financing operations- using a results-based financing instrument across the portfolio, supported by complementary technical assistance and analytics.

(a) Indonesia’s Supporting Primary Health Care Reform (I-SPHERE) aims to strengthen the performance and quality of Indonesia’s primary health care nationally, with an additional focus on Eastern Indonesia. It is a US$150 million program with a focus on supporting primary care reform by improving data use and accountability, strengthening the accreditation of primary health care facilities, and enhancing performance-based financing through JKN and inter-government transfers. (b) Investing in Nutrition and Early Years (INEY) is a US$400 million multi-sectoral program supporting the Government’s program to strengthen investment in activities that delivery priority nutrition interventions for maternal and child health and nutrition services, water and sanitation, early childhood education and development, and social protection. A second phase of INEY2 in an amount of US$ 600 million has been recently approved, to be implemented during 2023-2028, and is co-financed by Global Financing Facility and Gavi. (c) Indonesia Emergency Response to COVID-19 program is a US$ 750 million-dollar program, also supported by US$ 1 billion in co-financing from partners, that supports the Government of Indonesia in responding to the COVID-19 pandemic and rolling out a national COVID-19 vaccination program. It is co-financed by Asian Infrastructure Investment Bank, KfW and Government of Australia. (d) Indonesia’s National Health Insurance Reforms and Results Program, approved in December 2021, is a US$ 400 million-dollar program, also supported by US$ 2.33 million in co-financing from the Gates Foundation, that supports the Government of Indonesia in improving the efficiency and sustainability of the JKN program and leverage its influence for better quality of health services. (e) A new operation on strengthening the National Tuberculosis Response Program for Results (TB PforR) that was declared effective in January 2023 is focusing on improving accountability of the subnational governments in TB program, increasing the involvement of the private sector especially at the primary level of care, and the use of digital technologies. The PforR uses health system approach to address longstanding challenges of the TB program, and is co-financed by the Global Fund. (f) A Human Capital Development Policy Loan was approved in FY22 and supports several key health policy actions. (g) A new Health Systems Strengthening operation, focused on addressing health infrastructure requirements at all levels of the Indonesian health system, is currently under preparation for an expected World Bank financing of over US$ 1.4 billion, and is scheduled for approval in December 2023. (h) The analytical work undertaken through Reforms to Strengthen UHC in Indonesia program of analytical work supports the GOI’s efforts to accelerate and sustain progress towards UHC. The program addresses systemic challenges in the health sector’s governance and accountability, financing beyond JKN, and the quality-of-service delivery that hinder progress to UHC. The Global Fund, Global Financing Facility, Government of Australia, Government of Japan and the Gates Foundation co-finance this Programmatic Advisory Services and Analytics (PASA) and its technical assistance activities to support the aforementioned operations. (g) Multisectoral Nutrition program of analytical work provides analytical underpinnings and technical assistance support to implement a multisectoral convergence approach to addressing malnutrition and child development in early years. This program is supported by the Global Financing Facility for Women, Children and Adolescents, and by the Indonesia Human Capital Accelerator multi-donor trust fund.

The Indonesia HNP team also supports the World Bank HNP program in Timor Leste that is a post-conflict lower middle-income country with significant development challenges, including very high levels of stunting, and a growing portfolio of health and nutrition operations and analytical work.

The Indonesia and Timor-Leste HNP team is looking to hire an ETC-Health Specialist to support its growing portfolio of operations and analytical work.

Roles & Responsibilities:

The ETC- Health Specialist will report to the Practice Manager for Health, Nutrition and Population Global Practice responsible for the EAP HNP portfolio.

The ET Consultant will have the following key responsibilities, inter alia:

• As a team member, closely support health and nutrition lending operations and analytical work of the Indonesia health and nutrition team; • Contribute, support and facilitate the lending portfolio of the Indonesia HNP team, with a special focus on the Tuberculosis operation, and the related analytical work program, engaging with client ministries and internally within the World Bank; • Support the World Bank Indonesia Health team in delivering high quality technical assistance activities by tracking, facilitating and co-organizing the relevant capacity-building activities, including meetings, workshops, seminars, etc., with a special focus on the Tuberculosis PforR; • Organize and support implementation support missions of the assigned operations; • Support and contribute in analytical work related to the scope of the assigned operations • Partake in and contribute to technical dialogue with government counterparts— including at the national and sub-national levels—and with other relevant partners in relation to the assigned World Bank operations; • Undertake field missions to different parts of Indonesia as and when necessary, for implementation support as well as for analytical work related to the assigned operations • Support the Health team efforts in working closely with other GPs, the Country Management Unit, and external partners in the health portfolio; • Draft and review operational documents such as Aide Memoires, Implementation Status Reports, and updates to management and partners • Contribute to the development, writing and dissemination of case studies and papers on lessons learned with the Governments, Bank staff, and international partners • Contribute to the development of regular reports, management briefs, other formal communications using the World Bank system. • Respond to ad hoc information requests from internal and external parties. • Carry out any other activity required as part of the health and nutrition portfolio.

Selection Criteria

• Master’s degree in public health, epidemiology, health economics, health financing, public policy and management, business administration, or a professional clinical degree (medicine, nursing) Candidates without a core qualification in at least one of these domains need not apply. • Minimum of 5 years of relevant experience in one or more of the following areas: health systems, public health, epidemiology, surveillance, health economics, program management, service delivery, monitoring and evaluation, operations research in health. Experience in two or more of these areas is advantageous. • Experience with the health sector in Indonesia is highly desirable and will be an advantage. Experience working in communicable disease control, especially in Tuberculosis program implementation/management, will also be an advantage. • Prior experience of working with international organizations is desirable. • Excellent communication skills and fluency in English and Bahasa Indonesia is essential for this position • Ability to (a) deal sensitively in multi-cultural environments and to work in a team environment; (b) operate independently with minimal direct supervision and support; (c) work effectively and independently as a team member and as a leader alike, including in multi-sectoral teams; (d) share and disseminate relevant information with colleagues, clients, and managers; and (e) interact effectively with a range of partners within and outside the Bank, and to apply strong diplomatic and client-engagement skills.
