PURPOSE AND OBJECTIVE.

The COVID pandemic highlighted the importance and need for oxygen to treat COVID cases worldwide. In Mozambique, COVID has impacted the fragile health system with extreme oxygen unavailability, even for resuscitation use of newborns and/or mothers during birth/delivery. Despite efforts by MISAU and different partners to install "plants" and increase the availability of liquid oxygen or to acquire oxygen-related equipment - as UNICEF is doing, there is a gap in clinical guidance for using oxygen at different levels of health care.

The objective of this consultancy is to produce clinical guidelines for the use of oxygen for the main conditions (including maternal, neonatal and child health) adjusted to the different levels of health care.

METHODOLOGY AND TECHNICAL APPROACH.

Under the coordination of DNAM/Department of Specialized Medical Care at MISAU and UNICEF, the consultant the Consultant should:

• Use participatory methodologies and interviews with professionals from MISAU (DNAM) in the clinical and pharmacy area, Oxygen working group, Technical Group of Infant Health / DNSP, cooperation partners that deal with oxygen matters with the aim of gathering information regarding the constraints prevailing in the context of oxygen management at the level of the SNS;

•Listen to the different actors, namely Heads of Provincial Medical Assistance, Chief Medical Officers, Clinical Directors of health units, managers of the provincial, district and health units levels, as well as those responsible for the management of drug depots at provincial and district level to closely assess key aspects in oxygen management in the NHS;

•Consult the existing literature, periodic reports of Oxygen therapy including international oxygen therapy protocols;

•Update, improve and/or adapt oxygen therapy standards to current conditions;

In order to obtain subsidies for suggestions for improving services, the consultant should also:

•Listen to General Practitioners and various Specialties in relation to the criteria of oxygen use, for hospital use and for Primary Health Care;

•Listen to those responsible for drug deposits to measure oxygen demand at the various levels of action;

•Hold coordination meetings with the oxygen working group to be organized in close coordination of DNAM/DNSP -Health of children at MISAU and UNICEF.

The preliminary report is expected to be submitted to the leadership of DNAM, its partners with particular participation in the oxygen technical working group.

ACTIVITIES AND TASKS.

• Prepare the detailed work schedule of the consultancy;

• Review relevant literature for familiarization and adequacy of international standards and/or protocols for the use of oxygen;

• Conduct all stages of auscultation of the various actors provided for in the process;

• Prepare a report with the main findings, strengths, weaknesses, list of constraints, challenges and lessons learned about the current and real situation of oxygen use in the country; This report should also include a number of programmatic recommendations to be made and the approach to its implementation;

• Develop proposal for clear guidelines and criteria for the use of oxygen according to clinical specificities and level of care;

• Hold regular meetings with the Oxygen Technical Working Group to report the status of the consultancy, level of execution, main constraints and progress;

DELIVERABLES AND PAYMENTS.

Payments will be processed upon acceptance of the corresponding deliverable and against an invoice that will reference the contract and deliverable numbers. Payments will be approved by the respective section chief.

Deliverable/product(s): Relatório inicial contendo a análise da situação do uso de Oxigénio incluindo matriz de resultados analisados e recomendações

Payment: 20%

Deliverable 3:

Delivery timeframe: Apos 7 dias de trabalho

Deliverable/product(s): Realização da oficina para disseminação do Relatório da analise situacional e validação das recomendações. Submissão do Relatório da Oficina.

Payment: 10%

Deliverable 4:

Delivery timeframe: Apos 15 dias de trabalho

Deliverable/product(s): Submissão das normas Clinicas nacionais para o uso de oxigénio nos hospitais e Centros de Saude. Submissão de uma pagina de adenda para o Caderno de mapas de AIDI (atenção integrada das doenças da infância) neonatal e AIDI da Criança

Payment: 25%

Deliverable 5:

Delivery timeframe: Apos 10 dias de trabalho

Deliverable/product(s): Realização da Oficina de Validação das normas produzidas. Submissão do relatório da Oficina.

Payment: 10%

Deliverable 6:

Delivery timeframe: Apos 10 dias de trabalho

Deliverable/product(s): Submissão das versões finais das Normas Clinicas Nacionais para o uso de Oxigénio e Centros de saúde. Submissão das versões finais das adendas para o Caderno de mapas de AIDI (atenção integrada das doenças da infância) neonatal e AIDI da Criança. Relatório final da consultoria.

Payment: 25%

MANAGEMENT AND SUPERVISION.

O consultor estará sob a supervisão e monitoramento do especialista em saúde no UNICEF da Chefe do Departamento de Atenção Médica Especializada DNAM/MISAU.

QUALIFICATIONS AND SPECIALIZED KNOWLEDGE.

Academic qualifications. Medico Especialista em Medicina Interna ou Pneumologia.

Work experience. Pelo menos 5 anos de experiência; Experiência na elaboração e revisão de estratégias/ normas clinicas; Experiência na condução de análises situacionais; Experiência na organização/orientação de oficinas de trabalho com intervenientes de múltiplas áreas

Specific knowledge, competencies, and skills required. Domínio da Política Nacional de Saúde, Conhecimento dos protocolos Clinicos de referencia internacional.

Language skills. Fluência em Português e Inglês, falado e escrito.

Consultants and individual contractors are responsible for determining their tax liabilities and for the payment of any taxes and/or duties, in accordance with local or other applicable laws.

The selected candidate is solely responsible to ensure that the visa (applicable) and health insurance required to perform the duties of the contract are valid for the entire period of the contract. Selected candidates are subject to confirmation of fully-vaccinated status against SARS-CoV-2 (Covid-19) with a World Health Organization (WHO)-endorsed vaccine, which must be met prior to taking up the assignment. It does not apply to consultants who will work remotely and are not expected to work on or visit UNICEF premises, programme delivery locations or directly interact with communities UNICEF works with, nor to travel to perform functions for UNICEF for the duration of their consultancy contracts.
