Purpose of consultancy:

The main purpose is to strengthen the capacities of doctors and midwives on perineal trauma scanning and repair through conducting training workshops. The training will include both theoretical and hands on components focused on the basic and advanced prevention, diagnosis & management of perineal trauma at the time of childbirth.

Background:

Most women undergoing vaginal birth, experience some sort of genital tract laceration. The risk is increased for first-time mothers and is considered one leading cause of postpartum haemorrhage. Obstetric anal sphincter injuries (OASI) are severe vaginal birth complication that involves disruption of the anorectal complex. Women sustaining OASI are more prone to perineal pain, sexual dysfunction, rectovaginal fistula, anal incontinence, and psychosocial problems. Proper instant repair of OASIS reduces the risk of severe complications including anal incontinence.Accordingly, doctors and midwives should be able to classify perineal tears accurately. OASIS recognition, however, may be challenging for birth attendants which requires adequate well-structured training. A previous study from Palestine, showed that perineal injuries were underestimated, and that clinical training of birth attendants in the diagnosis and classification of perineal tears, resulted in significant increase in the detection rate of OASIS from 0.5% to 3.1% (p< 0.005) among primiparous women and 0.2% to 0.6% (p=0.002) among parous women. Also, doctors should receive proper training to improve their repair and management skills of OASI, to achieve the optimum results and quality of life for women. In Palestine, the educational programs for future specialists in obstetrics and gynaecology, lack a formal clinical training in correct technique of episiotomy and assessment and repair of obstetric perineal injuries, resulting in disarray of management protocols and clinical practice among different maternity units and between maternal care providers within the same organization, something that emphasizes the importance of delivering relevant training courses at regular basis.

At the end of the training, attendees will have the knowledge and skill to:

  • Recognize evidence-based interventions for the prevention of perineal trauma
  • Correct diagnosis of and classification of perineal injuries
  • Perform the continuous technique for episiotomy and second-degree tears repair
  • Undertake end-to-end & overlap repairs of obstetric Anal Sphincter injuries (OASIs)
  • Understand the principles of using 2D & 3D trans-perineal ultrasound (TPU) in assessing pelvic floor disorders
  • Appreciate the different TPU techniques to optimize image acquisition
  • Differentiate between normal and abnormal findings on static and dynamic imaging.

Work to be performed

A series of eight training workshops will be delivered, by international experts in the field of the prevention, diagnosis and management of complex perineal trauma. To ensure optimal supervision and guidance throughout the hands-on training, a maximum number of 20 candidates will be accommodated per workshop.

Output 1: Develop training materials and tools

Deliverable 1:1: Prevention, diagnosis and repair of perineal injuries training materials (trainer and trainees training tools), Power Points (e-learning materials if possible).

Output 2: Develop training plan.

Output 3: Conduct training workshops in line with the plan.

Deliverable 3.1: strengthen the capabilities on prevention, diagnosis and repair of perineal injuries.

Deliverable 3.2: Submit a draft Summary report along with key recommendations at end of the training activities.

Qualifications, experience, skills, and languages:

Qualifications required

Essential

  • Medicine university degree
  • Post graduate studies in Obstetrics and Gynecology

Desirable

  • Master’s degree in Public Health, or any relevant field.
  • Past experience of similar training.

.At least five years of experience in Obstetrics

  • Excellent knowledge of maternal health related activities, including situation analysis, planning, implementation, monitoring and evaluation.
  • Ability to write reports and implement projects adopting WHO recommendations on maternal health, focusing on childbirth complications, namely perineal injuries.

Medical clearance

Purpose:

Conduct on-site training

All travel arrangements will be made by WHO – WHO will not be responsible for tickets purchased by the Consultant without the express, prior authorization of WHO. While on mission under the terms of this consultancy, the Consultant will receive subsistence allowance.

Visas requirements: it is the consultant’s responsibility to fulfil visa requirements and ask for visa support letter(s) if needed.
